But another important component to helping women prevent breast cancer can be attributed to the foods we eat. “Good nutrition reduces the development of breast cancer and breast cancer recurrence,” shared <a href="https://boutiquebreastimaging.com/about-us/" target="_blank" rel="noopener">Dr. Kathryn Pearson, Founder and Director of Boutique Breast Imaging</a>. Here are some important nutritional tips Dr. Pearson wants women to know:</p><h3><strong>Nutrition to Prevent Breast Cancer:</strong></h3><p>It is estimated that one third of cancer deaths may be attributed to diet in adulthood. Conversely, good nutrition reduces the development of breast cancer. For breast cancer to “take hold” and grow, it depends on three mechanisms: (1) a cellular mutation creating a cancer cell; (2) an immune system that can’t fight it; and (3), the “feeding” of the breast cancer cell either through hormones or growth factors.  Extensive research has shown that breast cancer may be prevented or thwarted by targeting these mechanisms through nutrition (and other lifestyle factors).</p><p><strong>“<em>Let food be your medicine and medicine be your food.</em>” &#8211; Hippocrates</strong></p><h5><strong>1. Improve Immune Function</strong></h5><p style="padding-left: 40px;">a. Antioxidants – prevent oxidative damage in body cells</p><p style="padding-left: 80px;">i. Primarily plant-based diet: 15-25% reduction in breast cancer risk: <strong>Fruits </strong>(particularly berries, citrus, rosaceae fruits (apples, pears, apricots, nectarines, plums, peaches, cherries), <strong>vegetables </strong>(particularly carrots -decrease cancer cell production &#8211; and cruciferous vegetables – broccoli, cauliflower, brussel sprouts, turnips, cabbage), <strong>beans / legumes, nuts / seeds </strong>(flax seeds, pomegranate)<strong> and whole grains</strong>; <strong>green tea </strong>and <strong>spices </strong>(turmeric / curcumin)</p><p style="padding-left: 80px;">ii. Dietary antioxidants plus Multivitamin better than supplemental antioxidants (which may not be used in breast cancer patients during chemo / radiation),</p><p style="padding-left: 40px;">b. <strong>Vitamin D</strong> (want levels &gt; 50 ng/ml; each 1000 IU Vitamin D/day raises level approximately 10 ng/ml): 30% reduction in risk of breast cancer; recommended vitamin D dose (D3&gt;D2 for absorption) should be between 1,000-4,000 IU per day (and sometimes even higher).</p><p style="padding-left: 80px;">i. Turns on cellular apoptosis (kills bad cells), regulates cell growth / differentiation</p><p style="padding-left: 80px;">ii. Reduces mammary tumors in mice</p><p style="padding-left: 80px;">iii. Reduces estrogen effects in the breast (decreased density, decreased glandular activity)</p><p style="padding-left: 80px;">iv. Add Vitamin K2 (90 mcg daily) to improve calcium distribution to bones and teeth rather than arteries and soft tissues (and decreases urinary calcium excretion).</p><p style="padding-left: 40px;">c. <strong>Green tea</strong> (1-4 cups daily): 22% reduction</p><p style="padding-left: 40px;">d. Exercise (walking fast 30 minutes/day for 6 days/week), yoga: 50% reduction in morbidity and mortality from breast cancer</p><p style="padding-left: 40px;">e. Sleep (&gt;9+ hours: 33% reduction of breast cancer; from higher melatonin levels, protective)</p><p style="padding-left: 80px;">i. Decrease overnight-shift work where exposed to light</p><h5><strong>2. Reduce Inflammation</strong></h5><p style="padding-left: 40px;">a. Reduce sugars (limit processed and refined grains / flour / sugars)</p><p style="padding-left: 40px;">b. High fiber diet (beans / legumes, seeds, whole grains)</p><p style="padding-left: 40px;">c. Reduce exposure to micro-plastics</p><h5><strong>3. Reduce Chemical Exposure</strong></h5><p style="padding-left: 40px;">a. Organic foods (decrease pesticide exposure, increase antioxidants, increase salicylic acid, increase phytonutrients)</p><p style="padding-left: 40px;">b. Decrease pesticide exposure</p><p style="padding-left: 40px;">c. High fiber diets (bind to chemicals / carcinogens / bile acids which are then eliminated)</p><p style="padding-left: 40px;">d. Plenty of fluids, water, non-caffeinated beverages</p><p style="padding-left: 40px;">e. Green tea: blocks formation of cancer-causing nitrosamines</p><p style="padding-left: 40px;">f. Reduce animal fats, fried foods, smoked meats, charred foods</p><p style="padding-left: 40px;">g. Reduce alcohol (best to limit or avoid alcohol)</p><h5><strong>4. Reduce Estrogen Stimulation</strong></h5><p style="padding-left: 40px;">a. Limit alcohol intake (affects the liver, reducing breakdown of estrogen: increased circulating estrogen)</p><p style="padding-left: 40px;">b. Limit hormone replacement therapy after menopause if high risk / history of breast cancer / known genetic mutation</p><p style="padding-left: 40px;">c. Increased soy exposure as adolescent reduces breast cancer risk by 40%; WHOLE soy foods (not processed) may be protective in post-menopausal women (limit to 1-3 daily servings).</p><p style="padding-left: 40px;">d. Vitamin D reduces estrogen effects in the breast (decreased density / glandular activity)</p><p style="padding-left: 40px;">e. High fiber diets (reduce hormone levels; decrease obesity: decreased peripheral estrogen formation)</p><h5><strong>5. Reduce Insulin and Insulin-Like Growth Factor</strong></h5><p style="padding-left: 40px;">a. Maintain healthy weight (BMI &lt; 25 kg/m2) – Modest caloric restriction</p><p style="padding-left: 40px;">b. Low / moderate fat diet (emphasis on healthy fats, Omega-3 fatty acids)</p><p style="padding-left: 80px;">i. Low saturated fats (avoid red / pork meats, poultry skin, baked goods, whole milk products – butter, cheese, ice cream)</p><p style="padding-left: 80px;">ii. Low Trans-Fatty acids (avoid fried foods, processed foods, breads, crackers, cereals and cookies)</p><p style="padding-left: 80px;">iii. Increased Monounsaturated fats / Omega-9 Fatty Acids (extra virgin olive oil, avocadoes, almonds – limit nuts to ¼ cup/day)</p><p style="padding-left: 80px;">iv. Increase Omega-3 fatty acids (cold-water fish, DHA-enriched eggs, flaxseeds, chia seeds walnuts, pumpkin seeds)</p><p style="padding-left: 80px;">v. Low Omega-6 fatty acids (avoid vegetable oils, meats, egg yolks whole milk products</p><p style="padding-left: 40px;">c. Vegan protein sources (such as soy)</p><p style="padding-left: 40px;">d. 									<p>As of September 10th, the Food and Drug Administration is requiring notification of all <a href="https://boutiquebreastimaging.com/patient-care/" target="_blank" rel="noopener">mammography patients</a> that they do or do not have dense breast tissue, which is the patient-specific breast composition of dense fibroglandular tissue in proportion to fat tissue.  The reasoning for this is to appropriately educate patients with dense breast tissue that they may have (1) a breast cancer masked / hidden by the dense white breast tissue on the mammogram, and (2) a higher risk for developing breast cancer because of the prominent glandular tissue composition. Sadly, despite extensive awareness efforts, a recent <a href="https://urldefense.proofpoint.com/v2/url?u=https-3A__info.volparahealth.com_e_870741_lease-2Ddetail-2D-2DnewsItemId-2D25351_q16bns_837030953_h_7QSOSDXkDClSSIhVP-5Fb1TzZA9XHZjhUBPbwqjhvEw2E&amp;d=DwMDaQ&amp;c=euGZstcaTDllvimEN8b7jXrwqOf-v5A_CdpgnVfiiMM&amp;r=9lOHH5ChTEmuXKKi6l0JotsxlBlSfi0nSn3b7ShklkgchnuWvnKLhk75JESjbpXR&amp;m=AORgdQj5forkZfbS0uOViE1TfGCsGbrM5UTKtQV0pt1tBzacyEKAM1641G4RcKVd&amp;s=X9AgQisSl5nhzPE5tGYMxgP1Dwgz2TKbJGMZ1Xcdmgg&amp;e=">Myriad Genetics study</a> reveals that <strong>44% of women still do not understand breast density, and 63% are unaware of its connection to breast cancer.</strong> Ongoing patient education is essential, so, <em>What do I do if I have dense breast tissue? </em></p><p style="padding-left: 40px;"><strong>Look at your breast cancer risk </strong>through family history, previous biopsies and possibly indicated genetic testing: </p><p style="padding-left: 40px;">By combining genetic insights, family history and other clinical factors like breast density, MyRisk® Hereditary Cancer Test with RiskScore® calculates a woman’s 5-year and remaining lifetime risk of breast cancer. If a woman is found to be at high risk, she then has multiple options available to her, including a change in medical management, and additional supplemental screening imaging.  Our Boutique Breast Imaging center routinely assesses this Myriad MyRisk and provides a breast density-specific score to all patients under 70 years old. </p><p style="padding-left: 80px;">A. If <strong>high risk</strong> for breast cancer (greater than 20% Lifetime Risk for breast cancer or &gt;1.7% 5-year risk for breast cancer), then supplemental imaging with contrast-enhanced <strong>breast MRI is recommended every year</strong>, staggered 6 months from the mammogram.  Certainly, a known genetic mutation such as BRCA1 or BRCA2 would warrant an annual mammogram and breast MRI, as well as clinical evaluation twice a year with consideration for prophylactic measures (anti-estrogen treatments or even surgery).   </p><p style="padding-left: 80px;">B. If a patient has an <strong>intermediate risk</strong> for breast cancer (15-20% calculated Lifetime Risk for breast cancer), then the patient should <strong>consider additional Abbreviated Breast MRI imaging, particularly as a baseline, and / or Whole Breast Screening Ultrasound</strong> (more comprehensive if performed as a Handheld breast ultrasound by an experienced technologist/breast radiologist, versus an Automated Breast Ultrasound (ABUS), which may warrant repeat diagnostic ultrasound for questionable machine-detected abnormalities).  All of these additional studies for patients not meeting “high risk &gt; 20% lifetime risk” criteria are currently not routinely covered by insurance in the state of Florida and are offered at cash pay prices.   </p><p style="padding-left: 80px;">C. If a patient has an <strong>average risk</strong> for breast cancer (&lt;15%), then additional supplemental imaging should be considered in patients with <strong>Extremely Dense</strong> breast tissue (10% of all patients) with either an Abbreviated Breast MRI or Whole Breast Screening Ultrasound.  Additionally, in cases of <strong>Heterogeneously Dense</strong> breast tissue, those patients with higher anxiety levels or hormonal stimulation (i.e. peri-menopausal hormone surges or exogenous post-menopausal hormone replacement therapy) may consider additional imaging with Abbreviated Breast MRI or Ultrasound. </p><p>In conclusion, &#8220;<strong>Additional testing for women with dense breasts is medically necessary and the U.S. needs to catch up to other high-income countries that cover it. As a nation, we can do so much better&#8230;</strong>&#8221; Teri Thomas, Volpara Health CEO, was recently quoted in a USA Today article titled, <em>New mammogram guidelines from FDA shift what patients should know</em>. This was in response to Volpara Health, American Cancer Society, American College of Surgeons, and many others that support the <strong>Find It Early Act</strong>, a bill introduced in Congress in May 2023 that would guarantee full insurance coverage for women with dense breast tissue to get supplemental screening. 									<p><span style="font-style: inherit; font-weight: inherit; background-color: var(--ast-global-color-5);">I left the office thinking I&#8217;d take those over to my regular imaging center that afternoon to have the scans done hoping to alleviate my anxiety. I called the imaging center, and after waiting on hold, listening to Mozart&#8217;s Eine Kleine Nachtmusik for 15 minutes, I was transferred to the breast scheduling service. &#8220;No ma&#8217;am, today is not available. Our first available is Oct 4 in St Augustine,&#8221; said the scheduler.  It was September 4th. A little stunned at the timeline, I took the next available appointment that was closer to me, October 11. What&#8217;s an extra week with that kind of timeframe.</span></p><p>And then, I started to stew. How on earth was I going to wait 5 weeks for a scan? I could practically feel the tumor growing inside me. I was annoyed, verging on mad, and scared.</p><p>Sadly, I know several friends who have recently gone through breast cancer treatment over the last few years, so I reached out to one for advice. After offering some reassuring words, she suggested Boutique Breast Imaging and radiologist Dr. Kathryn Pearson, M.D. She knew Dr. Pearson from her own scans a few years prior and was going to contact her directly. In the meantime, I decided to call and see if I could <a href="https://boutiquebreastimaging.com/schedule/" target="_blank" rel="noopener">schedule an appointment</a>, though I remained skeptical.</p><p>After just two rings, a friendly, real voice came on the line. I explained my predicament, and the receptionist said not to worry, she&#8217;d get me on the schedule right away. In addition, she&#8217;d take care of getting the transfer of all my old mammograms from my previous imaging center. She would also call my general practitioner and ask that the imaging request come to her. There was nothing I needed to do but be there on Monday at 9 am. Seriously?</p><p>Upon arrival, I was welcomed by the receptionist and a knowledgeable technician who performed the mammogram. Afterward, I was taken to Dr. Pearson’s office to review the images. Dr. Pearson thoroughly examined everything, discussed the implications of dense breast tissue, and compared this mammogram with one I had from April. She then conducted an ultrasound, initially identifying a swollen gland but persisted in her examination. “I see something here, but I don’t think it’s cancerous,” she said. “There is some vascularity, though, and if it were me, I’d want to biopsy it.”</p><p>Though not the news I hoped for, I was immensely grateful for Dr. Pearson’s directness and her willingness to share her estimated risk of cancer. Her honest approach was reassuring, and she scheduled a biopsy for later that day. Throughout the process, she remained accessible to answer my questions and calm my fears, not even allowing me to wade into worst-case scenario waters.</p><p>If you’ve never had a breast biopsy, it’s not as bad as it sounds. There is some soreness afterward, but the procedure involves ample lidocaine, so the punctures are not felt. Dr. Pearson and her technician kept me at ease with friendly conversation, asking about my sons and my interests outside of work.</p><p>When the procedure was done, she explained that the lesion did not feel like cancer when the needle was inserted, though she knew it wasn&#8217;t a cyst. We wouldn&#8217;t know for sure until pathology reviewed it. And when I say we, I mean it. I felt like she was on my team and cared. She was even able to tell me that if it were cancer it would be stage 1 and curable. </p><p>Fast forward two days, and she left me a voice message. &#8220;I&#8217;m calling with good results. Everything is great but I do need to talk to you so please call me back.&#8221; Two minutes later I called back, and she <em><strong>answered.</strong></em> She went on to reiterate, that there was no cancer and no current treatment needed while also explaining what kind of lesion was present (PASH) and what kind of follow-up I will need in the future. </p><p>Stanford-educated Pearson, with an M.D. from UC San Francisco, said this is a passion project for her, and you can feel that.  I went to her<a href="https://boutiquebreastimaging.com/" target="_blank" rel="noopener"> website </a>to do some digging about her, and I see <a href="https://boutiquebreastimaging.com/media/" target="_blank" rel="noopener">she&#8217;s made breast radiology a life-long mission.</a> </p><p><strong>The service and compassion were an absolute A+, and I truly cannot imagine going anywhere else for my breast imaging needs.</strong>  Even if my diagnosis had been cancer, I feel very confident that Dr. Pearson and BBI would have thoughtfully guided me through the next steps. This is <strong>concierge-level medicine at its best</strong>. And yes, they do take most major insurance carriers including Medicare. Although my insurance was not covered (unfortunately United is not yet contracted with them, though BBI is working on it), they have affordable self-pay options. Moving forward, I will only do my <a href="https://boutiquebreastimaging.com/mammograms/" target="_blank" rel="noopener">annual breast mammogram at BBI</a>, self-pay or not. </p>								</div>

<h2 class="wp-block-heading has-large-font-size">How Does Boutique Breast Imaging (BBI) Provide Better Care?</h2>



<p>Boutique Breast Imaging is owned and operated by Breast Imaging Radiologists who want to take back control and provide superior patient care. The breast radiologists have acquired the best equipment, technology and staff to provide the best imaging quality. The breast radiologists take the time to talk to and examine the patients, show patients their images, discuss tailored screening and follow-up recommendations, and provide patient education. The breast radiologist understands the high level of anxiety that comes with breast cancer screening and does his/her best to schedule the diagnostic mammogram, ultrasound, biopsy and staging MRI all within a week, providing a surgeon with all reports, pathology and imaging needed for treatment planning and surgery scheduling, and providing the patient with “answers” and “expectations” that alleviate anxiety and fear. Most importantly, the breast radiologist has been subspecialty trained, beyond board certification for the American College of Radiology, to diagnose breast cancer and other issues of the breast, thereby providing the best and most accurate care for patients.</p>



<p>True Life Patient Examples:</p>



<p>1) A 25-year-old patient comes in with a palpable lump and undergoes an ultrasound. When the ultrasound technologist does not find any abnormality and reports this to the radiologist, (and most general radiologists would have sent the patient away to follow up with their primary care physician), the breast radiologist does a physical exam on the patient and targets the ultrasound to the concerning abnormality, diagnosing an invasive breast cancer. That young patient might not have survived her breast cancer had she been delayed significantly in diagnosis and treatment.</p>



<p>2) A 45-year-old woman seeks a second opinion after a Recall abnormality is evaluated as “normal” by a general radiologist. A first look at the images by the breast radiologist reveals a breast cancer, subsequently targeted and detected on ultrasound. That patient would have had a delay in breast cancer diagnosis, not able to undergo just a lumpectomy and no chemotherapy, as she successfully did.</p>



<p>3) A 37-year-old woman is recommended for a biopsy at an outside imaging center (by a general radiologist). The breast radiologist performs a few additional mammogram images with ultrasound and determines that the “suspected lesion” is benign and related to previous trauma (classic fat necrosis). No biopsy is needed.</p>



<p>4) A 60-year-old woman with history of breast cancer seeks follow-up imaging, and learns from the breast radiologist that her best method for screening for her dense breast tissue includes supplemental screening breast MRI. Her breast radiologist detects an early second malignancy in her opposite breast, requiring minimal surgery and no chemotherapy.</p>



<p>5) An 82-year-old woman is talked into undergoing a screening mammogram (she thought she “was all done with those mammogram exams”!) and a very small cancer is detected and removed surgically (small lumpectomy outpatient surgery), with no need for additional treatment other than an anti-estrogen pill. Had she ignored the mammogram, that same cancer likely would have grown to need extensive surgery and treatment (difficult to undergo at her age), significantly disrupting and/or shortening her life.</p>



<p>6) A 56-year-old woman presents for an ultrasound of her lymph nodes in her axilla, prior to undergoing a skin cancer removal in that upper extremity. Although “not part of the examination”, the breast radiologist reviews her recent screening mammogram (called “normal” by a general radiologist) and detects a highly suspicious lesion in the opposite breast, subsequently biopsied as a Stage 1 breast cancer, curable.</p>



<p>7) A 35-year-old African-American woman undergoes a diagnostic mammogram and sonogram for a palpable lump that appears characteristic for a benign fibroadenoma, commonly identified in African-American women. However, her myGeneHistory survey and risk calculation performed at intake indicated a 25% lifetime risk for breast cancer (which is high) and a recommendation for genetic testing. The woman agreed to undergo genetic testing (saliva test) while at the breast center, determining that she had a BRCA2 gene mutation that significantly increased her risk for breast cancer. Genetic counseling was offered in addition to her being recommended for annual breast MRI screening as well as entered into a High Risk Screening program with a breast surgeon and oncologist for close evaluation and offerings of risk-reducing medications or surgery. Had this patient just been seen by a general radiologist / general imaging center, her mutation may have never been detected and potentially life-saving recommendations made.</p>

<p>Early detection decreases breast cancer deaths. For women of average risk, the American College of Radiology / Society of Breast Imaging recommends annual screening mammography starting at age 40. Women at higher-than-average risk should, in general, start surveillance with mammography at an earlier age and consider supplemental screening as well.</p>
<p><span style="font-size: 1.71429rem; font-style: inherit; background-color: var(--ast-global-color-5);">Population Subgroups at Higher-Than-Average-Risk</span></p>

<p>Multiple factors increase risk for breast cancer: genetic mutations, family history, chest radiation at a young age, personal history of breast cancer or high-risk lesions, breast density, and race/ethnicity. Genetic abnormalities are found in only 5% to 10% of breast cancers, with BRCA mutations the most recognized. The lifetime breast cancer risk is 72% among BRCA1 carriers and 69% among BRCA2 carriers. All women should undergo risk assessment by age 25, especially Black women (who are at risk for breast cancers at a younger age) and women of Ashkenazi Jewish heritage, so that those at higher-than-average risk can be identified and appropriate screening initiated.</p>

<p>For most women at higher-than-average risk (&gt;20% Lifetime Risk), the supplemental screening method of choice is breast MRI. For women with dense breasts who desire supplemental screening, breast MRI is recommended. For those who qualify for but cannot undergo breast MRI, contrast-enhanced mammography or screening breast ultrasound could be considered</p>
